I. APPLICATION GUIDELINES/FORMS
CEARP Guidelines/Application Packet
is available at no cost. Packet includes: Guidelines Overview;
Faculty Directed, Independent Study & Provider application instructions and
forms; Appendices and Application Fees.
II. CEARP Fee Schedule for Faculty Directed and Independent Study Applications
(for two year approval period)
Other CMAs, staff development departments, colleges or universities, commercial
education providers or planners: $100 (3 contact hours or less); $100 + $10 for
each contact hr. over 3.0 or the equivalent of course registration fee charged
to RNs, whichever is greater.
WSNA Districts and Local Units: no fee if 3 contact hrs. or less; flat fee of $50
for Faculty Directed Activity over 3.1 contact hrs. and for all Independent Study
applications.
Rush Fees: Additional fees are required for applications submitted less than
45 calendar days before the planned educational activity according to the schedule
below:
44-41 days ------------ $750.00
40-31 days ------------ $1000.00
30-21 days ------------ $1500.00
20-11 days ------------ $2500.00
No applications will be accepted for educational activities planned to occur fewer
than ten days before the application reaches the WSNA offices.
III. CEARP Fee Schedule for Provider Unit Application
(for three year approval period)
WSNA Districts and Local Units: $600
Other CMAs, staff development departments, colleges or universities, commercial education
providers or planners: $900.
Extension Fee: $150 for each 60-day extension (120 days possible) for subsequent
approvals submitted less than 3 months before last date of month of expiration.
Must request extension in writing and obtain approval, plus pay extension fee. Expiration
date remains original date of expiration.
IV. Consultation
Phone consultation: no charge.
$100 / hour at WSNA.
$100 / hour plus travel/accommodations at applicant facility.
V. CEARP FEE POLICIES
- The application fee is nonrefundable once the review process has begun.
- The application fee submitted less an administrative fee of $100
is refunded if the application is withdrawn before review of the application begins.
Fees are not refunded for offerings reviewed and denied approval.
