CEARP Guidelines & Application

The CEARP Guidelines/Application Packet is available for download here. Please carefully follow the instructions below to ensure that you download and complete the correct packet.

It is each applicant's responsibility to see that they use the correct and most recent version of the CEARP Guidelines.

Please note: The documents contained in the CEARP Guidelines/Applications Forms packets are in Microsoft Word format. If you do not have Microsoft Word installed on your computer, you may download and install the free Microsoft Word Viewer.

Step 1

There are three types of CEARP Guidelines/Application Forms packets. Download and read through the CEARP Overview first to determine eligibility and which type of packet you need. Reading the Overview is essential, as it includes important information about the review and approval processes, the ANCC Educational Design Criteria, and tips for adhering to these criteria.

Step 2

After determining which type of packet you need, download and complete all the documents listed below for your packet.

To download and save the forms, right click on each link and select "Save target as..." or "Save link as..." Then, select a location to save the forms, and click "Save."
