File a Staffing Complaint / Assignment Despite Objection (ADO) Form
What Is an ADO Form?
An ADO (Assignment Despite Objection) form is a formal way to report conditions that impact:
- Patient care
- Nurse safety
- Workplace satisfaction
It shows you made a good faith effort to resolve the issue and alerts leadership to ongoing problems.
Why File One?
- Document unsafe conditions
- Protect your license
- Start a process for solutions
- Hold management accountable
- Support your union in identifying trends
- Can trigger a DOH/L&I investigation if not resolved
When Should You File?
Think S.O.S. –
Short on Safety – Supplies – Staff
File if:
- Unit is understaffed or not following the staffing plan
- Charge nurse is pulled into patient care without support
- You’re given an unsafe patient load
- You’re assigned to a unit or patients you’re not trained for
- Mandatory OT, missed breaks
- lack of supplies
- Unsafe or missing equipment
Before You File:
- Clarify what is being asked of yourself regarding your assignment
- Assess your capabilities, obstacles, acuity of patients, and availability of ancillary staff
- Identify all possible options for meeting the patient’s needs
- Decide, communicate the decision to your Charge Nurse or immediate supervisor, and document the decision-making process

- Include your manager’s email so they are notified
(The form won’t be reviewed by management without it!)
No Retaliation Allowed
Your right to file is protected by:
- WSNA contract
- State law
If you experience retaliation, contact a WSNA Local Unit Officer or Nurse Rep immediately.
