Staffing Complaint Forms - ADOs (Assignment Despite Objection)
Did you know you can use an online ADO form to document any unsafe working condition? That includes: being understaffed to the matrix, inadequate support staff, workplace violence, missed breaks, not enough break nurses to cover breaks, being forced to use buddy breaks, equipment malfunction, lack of needed supplies and more. This form lets management and the union know that things are not okay.

Report your staffing concerns in real time to your charge nurse or manager to give them an opportunity to correct the situation. If not corrected, fill out the ADO to document the issue.
