Your nurse rep, Jaclyn Perkins, is now Jaclyn Smedley and has a new email address for you to reach her. For any issues needing to be addressed, please email [email protected]. The old email address ([email protected]), will remain active for some time, and any emails coming to this address by accident will be forwarded to the new address. However, we do ask for all nurses to update their contacts with the most current information so not a single email is at risk of being missed. Other information such as office numbers etc. remain the same.
